Huh. This was two years ago but I didn't notice it until now: "O'Reilly Releases DocBook: The Definitive Guide Under the GNU License".
Almost makes me want to try again to get Jade to work.

The XSL templates are pretty good - my big mistake was not validating against a DTD, I'd made lots of incorrect assumptions about what elements could go inside what... and then was wondering why the format didn't come out right.
